To avoid blood sugar surges, Agatston created a modified carbohydrate plan, recommending plenty of high-fiber foods, lean proteins, and healthy fats, while cutting bread, rice, pastas, and fruits. Major differences from other diets include a lack of concern over portion size and a serious indifference to exercise. Feeling full while on a diet is a beautiful thing, but it seems odd that a cardiologist buries his exercise recommendations in a solitary sentence. </p><p>  The last half of the book covers his three-stage plan; daily diets are mixed with recipes, some of which are from South Beach restaurant chefs. The most restrictive period lasts just two weeks, enough time to stabilize your urges and lose a few pounds; stage two adds fruits and a handful of other carbs, while stage three is meant to last the remainder of your life, with occasional lapses for white bread or birthday cake.  While the diet is sound, the book could be better organized. The first half mixes scientific study with anecdote in a seemingly random way, while the mix of meal plans and recipes can be confusing. He shows us how these unproven hypotheses were emphatically embraced by authorities in nutrition, public health, and clinical medicine, in spite of how well-conceived clinical trials have consistently refuted them. Obesity is a disorder of excess fat accumulation, not overeating and not sedentary behavior. <br>6. Consuming excess calories does not cause us to grow fatter any more than it causes a child to grow taller. <br>7. Exercise does not make us lose excess fat; it makes us hungry. <br>8. We get fat because of an imbalance&#8212;a disequilibrium&#8212;in the hormonal regulation of fat tissue and fat metabolism. More fat is stored in the fat tissue than is mobilized and used for fuel. We become leaner when the hormonal regulation of the fat tissue reverses this imbalance. <br>9. Insulin is the primary regulator of fat storage. When insulin levels are elevated, we stockpile calories as fat. When insulin levels fall, we release fat from our fat tissue and burn it for fuel. <br>10. By stimulating insulin secretion, carbohydrates make us fat and ultimately cause obesity. By driving fat accumulation,  carbohydrates also increase hunger and decrease the amount of energy we expend in metabolism and physical activity.<br>11. You'll also find an expanded chapter on alternative therapies that encompasses Ayurvedic and traditional Chinese medicine, along with a hefty section on pain control that offers a grab bag of options such as acupuncture, biofeedback, guided imagery, chiropractic care, and massage.<p> Still, the bulk of the book remains the more than 250 health conditions--from everyday problems such as insect bites and bad breath to serious diseases including bulimia, cancer, and AIDS--and the nutritional protocols the Balches recommend for treatment. Since any number of supplements can be taken for the same condition, the Balches make sifting through the glut of information a little easier by separating their nutrient recommendations into four categories: essential, very important, important, and helpful. And they take a lot of the guesswork out of buying supplements by listing the brands they know and trust. Once again, the authors have squeezed in an impressive amount of information, including valuable sidebars on topics such as the dangers of aspartame; how to choose a calcium supplement; common heart problems and procedures; cancer risk factors, diagnosis, and treatments; and sports nutrition. This is not relaxing reading, but it's enormously useful. According to Sarno, the mind tricks you into not facing repressed emotion by making you focus on pain in the body.
